Empowering Individuals Through Rental Assistance and Housing Solutions
The Homeless to Home (H2H) program supports working individuals and families experiencing housing insecurity by offering two vital services: rental assistance and eviction prevention. Through rental assistance, participants can receive up to $1,000 to help with move-in costs or monthly rent, making it possible to secure safe and stable housing. Eviction prevention provides up to $1,000 for those at risk of losing their current housing, helping them remain in their homes and avoid homelessness. Together, these services create pathways for stability and a stronger foundation for the future.
Are you currently unhoused and need Rental Assistance to MOVE into Housing?
This program supports individuals who are currently unhoused but have secured a permanent place to live. Assistance helps cover move-in costs such as deposits or first month’s rent.
Who is Eligible?
- Currently unhoused (e.g., living in your car, couch surfing, staying on the stretse)
- Have a signed lease agreement with a verified property owner or management agency (no subleases, transitional housing, or weekly rentals)
- Employed for at least 30 days
- Earning between $1,100 and $3,000 per month after taxes (employment income only)
- Hold a valid government-issued photo ID
Are you currently housed and need Rental Assistance to STAY in your Home?
This program supports individuals who are currently housed but at risk of eviction due to extenuating circumstances such as medical emergencies, loss of job (now back working), reduction in work hours, or other unforeseen challenges.
Who is Eligible?
- Currently housed but facing eviction or an immediate risk of homelessness
- Able to provide proof of hardship (e.g. eviction notice, medical documentation, employer letter)
- Employed for at least 30 days
- Earning between $1,100 and $3,000 per month after taxes (employment income only)
- Have not received H2H assistance in the last 6 months
- Hold a valid government-issued photo ID
How the H2H Program Works

Upload Required Documentation
After creating your profile, upload all necessary documents through your application portal.
Assistance Disbursement & Case Management
If approved, financial assistance is issued directly to your leasing office. Participants also receive continued support and access to relevant CARE Complex services.
- Payments for mandatory courses are non-refundable once submitted, even if the application is later rejected.

- NOTE
Applications are reviewed in the order they are received. Currently, due to high application volume, the review process may take up to 6–8 weeks from the time all required documents are submitted in the portal. All updates will be provided via email and the online portal.
This program is not emergency-based. For immediate housing needs, please refer to the local resources in your area.
Incomplete applications will not be reviewed. Applicants who do not meet eligibility requirements may still access other CARE Complex services such as hygiene support, meals, and mail assistance. We reserve the right to decline applications that, even if complete, do not align with the goals and intended scope of the program.
